What Exactly is Eczema?

What exactly is Eczema? Simply put, eczema refers to several different conditions that cause skin to become irritated, scaly and rough and itchy. There are several types of eczema, but the most common is atopic eczema, closely associated with allergies.

The most common symptoms of eczema are scaly, dry, itchy patches of skin. Depending on the sufferer?s normal skin color, the affected area may appear red, brown, or lighter or darker than usual. The skin affected by eczema usually seems thicker than unaffected skin, and any area can be affected. Upon visual examination, a doctor can often diagnose eczema. Since the condition is often associated with allergies, many doctors will review family history and usually request that the patient be undergo allergy testing. Blood tests and scraping tests are also used to diagnose eczema.

The most common types of eczema are caused by the body?s overreaction to some irritant. A reaction similar to an allergic reaction occurs, and the body?s immune system responds by fighting the irritant. Eczema occurs when the immune system overreacts, producing irritated, itchy skin. Eczema also reduces the skin?s ability to maintain moisture.

The itching carries a risk of infection, so antihistamines are often prescribed to control the itching and prevent infection. Topical lotions and creams are recommended to keep the skin moist, and hydrocortisone creams are used to reduce the inflammation. Other treatments for more severe cases of eczema include tar treatments, oral steroids and light treatments. Although new prescription drugs have been developed to treat eczema, they have been linked with an increased risk of cancer, and doctors have been warned to prescribe cautiously.

Although it may appear so, eczema is not contagious. The disease has no known cure, but can be managed effectively with prescription medications and topical lotions and creams.

The Benefits of Invisalign Over Metal Braces

Weight Loss Information Setting Effective Goals

Weight loss information is often confusing and misleading. The more specific we are in discussing weight loss information the better it is for everyone. Here's an example...realistic goals. What are realistic goals? I wrote an article not to long ago that defined realistic as a 5-10% reduction of your total body weight. The 5-10% reduction is all the weight you need to lose to show a health benefit or improvement.

But I think there is a more specific term that can be used when disseminating weight loss information, specifically dieting goals. How about Effective Goals. What are effective goals? Effective goals are:

1) Specific and measurable
2) Attainable
3) Forgiving

Exercise more is a great idea, but it's not specific. Walk 1 hour everyday is specific and measurable, but is it attainable? Maybe, maybe not. Walk 1 hour 3 times per week is a great goal. It's specific, measurable, attainable, and forgiving (leaves room to miss a day).

A behavioral technique, called shaping, utilizes this concept of effective goals, but makes it even better. It uses only small short-term goals that get closer and closer to the ultimate goal. It is based on the concept that nothing succeeds like success. Shaping uses two important behavioral principles:

1) Consecutive goals that move you ahead in small steps are the best way to reach a distant point.

2) Consecutive rewards help to keep you motivated.

I am going to start using Effective Goals instead of realistic ones. So, your weight loss information for the day...set effective goals and shape them.

Exercises For Lower Abdominal Toning

Fitness is synonymous with a flat lower abdominal region. Regarded as the aesthetic center of the body, the abdominal region is of vital importance to health and well being. It is important in digestion and stability of the lower back. The muscles of your midsection are not isolated; they weave through your torso like a web of tissues and all muscles need to be trained in a balanced manner. When your abdominal muscles are weak, the muscles of the buttocks (the gluteus maximus) and the back of the thighs (the hamstrings) have to work harder to keep the spine stable.

The secret to developing great abs lies at least partially in ignoring conventional wisdom. For starters, treat your body as a complete unit rather than over emphasizing the waistline. The most common mistake made by most individuals is doing hundreds upon hundreds of abdominal exercises, but neglecting other calorie burning cardiovascular exercises. This includes walking, cycling, swimming, jogging etc. Strength training for the lower and upper body boosts metabolism, which is critical to low body fat.

THE 3 KEYS TO A TIGHTER MID-SECTION.

1. Eat A Low-Fat Diet.

2. Regular Aerobic Exercise.

3. Perform Specific Abdominal Exercises To Firm And Tone The Abdominal Muscles For 5 Minutes Everyday.

THE BEST EXERCISES FOR LOWER ABDOMINAL TONING

The following exercises must be done 4-5 times a week, and the entire routine takes 6-8 minutes to complete. For best results, start with 15-20 repetitions and one set. Increase gradually to 2 sets. These exercises can be done at home, without cost and equipment - this is a free home abdominal workout! Complete the routine at least 2 times a week and you will see results within the first few weeks. These numbers are general guidelines. For best results, consult a personal trainer. If you have any injuries or medical ailments, please obtain a physicians clearance before starting any exercise program.

Reverse Curls ? Lower Abdominal Muscles.

Starting Position: Lie on your back with your hands at your sides. Raise your legs straight up in the air. Movement: Exhale as you bend your knees while curling your lower body towards your chest. Inhale as you slowly return your legs to the starting position. Remember to squeeze your abs, and repeat as required.

Lying Scissors ? Lower Abdominal Muscles.

Starting Position: Lie on your back on an exercise matt with your palms on the mat under your lower back and your legs outstretched. Movement: Exhale as you alternately raise your legs up in the air with a slight bend in them. Inhale as you lower your legs back down to the matt. Repeat as required.

Side Crunches ? Oblique Abdominal Muscles.

Starting Position: - Lie on your back with your knees bent together, and your waist twisted so that your legs lie on top of each other to one side of your body. Make sure your shoulder blades are evenly flat against the floor. Movement: - Exhale as you bring your upper body off the ground. This involves a contraction of your oblique abdominal muscles. Repeat the required repetitions. Switch your legs to the other side of your body to work the opposite side.

Abdominal Crunches ? Upper Abdominal Muscles.

Starting Position: Lie flat on your back. Bend your knees and hips. Place your feet flat on the floor. Gently clasp the hands behind the head, with the elbows pointing outwards. Movement: Inhale before you begin. Exhale as you curl up slowly pulling your head, neck, shoulders and upper back off the floor. Remember to keep the chin away from the chest. Keep your lower back on the floor throughout the exercise. At the upper limit of this movement, crunch or squeeze the abdominal muscles by holding the position for 1 - 2 seconds. Slowly release the curling motion, and inhale as you return to the starting position. Repeat as required.

Bicycle Crunches ? Oblique Abdominal Muscles.

Starting Position: Lie on your back on an exercise matt with your knees bent and hands behind your head. Raise your knees into the air. Movement: Exhale as you alternately bring your elbows towards the opposite knee while crunching your upper body off of the matt. Inhale as you lower your legs back down to the starting position. Repeat as required.

Lying Face Down Plank ? Lower Abdominal And Lower Back Muscles.

Starting Position: Get on an exercise matt on all fours, with your elbows touching the matt directly underneath your shoulders. Extend your legs back as far as you can, and keep the toes on the floor. Movement: Raise the hips up and hold yourself in this ?plank position? with your back completely flat. This is an excellent exercise for the hips, thighs and abdominal muscles. Since it involves so many muscles, it is strenuous in nature. Lower the hips slowly down to the mat. Repeat for 15-20 repetitions. (Most individuals find it hard to complete 15-20 reps for this exercise and 8-10 might be a more suitable starting point).

10 TIPS TO SUCCEED IN YOUR ABDOMINAL TRAINING PROGRAM

1. Get a medical evaluation and a complete physician?s clearance before beginning a fitness program. This is especially important if you're over 35, have been sedentary for a long time, have high blood pressure and/or cholesterol, are a smoker, or have chest pains or shortness of breath.

2. Eat a healthy, balanced diet at all times. Avoid gimmicks and fad diets.

3. Start your days smart by working out before breakfast. Research has shown that those who exercise in the morning are most likely to stick with an exercise program.

4. Do not do too much too soon. Try to stay within your limits, and only do what can be sustained.

5. Have a specific objective in mind when starting a new routine. Everybody wants to lose weight and get in shape. Try and reduce your waist measurement by 1 inch in the next 6-8 weeks with the strategies in this article.

6. The fitness program must fit your life. You should be comfortable with what you do and enjoy it.

7. Challenge your body every single time! Mix up the routine.

8. Get a workout partner, someone who motivates you.

9. Always warm up and cool down.

Astounding Information on Soft Drinks and Their Impact on Our Health and Wellness

Soft Drinks, The regular stuff!

We all know sugar sweetened soft drinks are empty calories and help contribute to the overall obesity problem in the western world. What most of us don't know is how much of an impact these have and how switching to diet drinks may be an even worse choice.

In 2000 Americans alone spent $60 billion on carbonated soft drinks and billions more on non-carbonated fruit beverages. During the same year the average American consumed more than 53 gallons of soft drinks. (Source: National Soft Drink Association). Those 53 gallons are equal to over 283 12 ounce cans each on average.

In 2004, soft drink companies produced enough non-diet soda to provide each U.S. resident with about 395 12 ounce cans, and enough diet soda for about 160 cans each, says the Center for Science in the Public Interest.

North American consumption represents more than 44% of worldwide consumption, Europe, with a per capita average of 12.7 gallons per year, represented 31%. The fastest growth in soft drink consumption is in Asia and South America. (Source Beverage Marketing Press Release June 15, 2001).

During the late 1950's the typical soft drink order in a casual or fast food restaurant contained about 8 ounces of soda. Today a child order of Coke at McDonalds is 12 ounces. A large Coke is 32 ounces and about 310 calories.

The average 12 ounce can of soda contains about 40 grams of refined sugar. That's about 8 Teaspoons! Did you ever consider you were eating 8 teaspoons of sugar with every can of soda pop? For comparison the USDA recommends that for a 2,000 calorie per day diet we should not consume more than 40 grams of refined sugars per day. In addition each can contains about 150 calories with zero nutritional value per 12 ounce can.

You can see how an extra 300 to 400 calories per day can contribute to weight gain.

Dr. Caroline Apovian, an associate professor at the Boston University School of Medicine and director of the Weight Management Center at Boston Medical Center, says one study found that the chance of becoming obese increased 1.6 times for each sugar sweetened drink consumed per day.

In addition to the calories, the sugar and the impact on weight gain and obesity, soda contains caffeine. Caffeine as we know is a mildly addictive stimulant drug that keeps us coming back for more. Research has shown that the caffeine delivered in a single 12 ounce can of soft drink is enough to produce mood and behavioral effects. Fruit Drinks:

Like I said repeatedly in my book Living To BeYoungerwe are literally being marketed to death. I go into great detail about how ad agencies and food companies are packaging products as a healthy alternative to junk foods.

There is no better example than the healthy alternative fruit drinks marketed by the soft drink companies. These companies market non-carbonated drinks that pretend to be rich in fruit juice and nutrients, but are basically sugar water. Coca-Cola Co.'s Fruitopia and Hi-C, for example contain only 5% to 10% fruit juice but they contain as much (or more) sugar as the carbonated soda pop. Don't be fooled by the claims of added vitamins and minerals on these products packages, they are still junk food.

The sweetener commonly used in soft drinks and other foods may lead to more body fat than drinks sweetened with plain sugar. A new study shows that fructose may alter the body's metabolism in a way that prompts it to store body fat.

Fructose is a sweetener found naturally in fruits and honey and is widely used as a sweetener in soft drinks, fructose is usually found in the form of high-fructose corn syrup which contains 55% fructose.

Researchers say the findings may help explain the recently established link between rising soft drink popularity and obesity rates in the United States and other parts of the world.

In the study, researchers led by Dr. Matthias Tschop of the University of Cincinnati compared the effects of feeding mice fructose-sweetened water, a soft drink flavored with table sugar, a diet soft drink or water. The mice were allowed to drink as much as they wanted of their assigned beverage.

The mice that drank the fructose-sweetened water gained significantly more body fat than the others, even thought they decreased the amount of calories they ate from solid food.

We were surprised to see that mice actually ate less when exposed to fructose-sweetened beverages and therefore didn't consume more overall calories, says Tschop, Nevertheless they gained significantly more body fat within a few weeks.

Body fat increased 11% in the fructose group, 5% in the water group, 7% and 8% in the diet and sugar water groups.

Researchers say the findings suggest the body metabolizes fructose differently than other sweeteners or carbohydrates and in a way that favors fat storage.

Between 1970 and 1990 high-fructose corn syrup consumption increased by more than 1000% largely because the nations soft drink producers switched from sucrose (sugar) to high-fructose corn syrup.

Making syrup is the second largest use of corn in North America, feeding livestock is the largest. On average Americans consume 48 pounds of corn syrup a year. (Source Anan Duming, Junk Food, Food Junk World Watch September / October 1991)

A 2004 report showed that Americans eat 132 calories each day of high-fructose corn syrup and that figure is closer to 300 for the top 20%of Americans.

Number who are obese goes from 12% to 17.9%.

The number of Americans who think about dieting and losing weight is probably at an all-time high. But surprisingly, so too are the number of Americans who are overweight (BMI >25) or obese (BMI >30): at least 30% of the population. The percentage of children and adolescents who are overweight has doubled since the early 1970s, raising concerns for long-term health effects. Excess weight is a health risk for cardiovascular disease and diabetes. By mid-1999, there were 7.1 million physician visits related to weight concerns.

Let's see high-fructose corn syrup production up 1000% from 1970, obesity and overweight doubled since 1970? Tripled in England during the same period?

Microbiotic Diet A Low Fat Way of Dieting

There are many diets to consider when you wish to change your eating habits, lifestyle, health, and weight.

Beans

The most common cooking oil used when preparing meals is dark sesame oil. Additional oils to consider include light sesame oil, corn oil, as well as mustard seed oil.

Flavourings

Natural sea salt, shoyu, brown rice vinegar, grated ginger root, fermented pickles, roasted sesame seeds and sliced scallions are some of the seasonings that can be used when adding flavor to foods.

When it comes to animal by-products while on the microbiotic diet, small amounts of fish or seafood are acceptable each week.

Dieters should stay away from eggs, dairy, meat, and poultry.

When eating fish or seafood, microbiotic dieters should consume horseradish, wasabi, ginger, or mustard in order to aid in the detoxification process against the effects of the seafood.

The Subconscious Diet Can Help Protect Your Child From Type 2 Diabetes

Until obesity became epidemic, type 2 diabetes was virtually unheard of in children and teens. It was originally called ?adult-onset? because it was mainly seen after middle age. At least 9 million American children are seriously overweight and another 9 million are heavy enough to be at risk, yet studies show that many parents are in denial about their children?s obesity.

16% of U.S. children have weight problems but doctors make weight reduction suggestions to parents about their children in only 1% of visits. There are a number of reasons for this denial and lack of concern:

Obese mothers nearly always recognize that they are overweight but only 1 in 5 correctly identified their overweight children.

Many doctors are just too busy to become involved in problems not directly related to the symptoms the child was brought in for.

78% of the population of the U.S. does not see being overweight as a health problem.

Many cultures equate thinness with poverty and starvation; to them a fat child is a strong and healthy child.

Many parents believe that the child will grow into their weight.

A child that is overweight will very likely become an obese adult. Studies show at least 15 conditions causally related to obesity. They include diabetes, heart disease, sleep apnea, hip fractures, high cholesterol, high blood pressure, end-stage renal disease, liver disease, urinary incontinence and several forms of cancer.

For many overweight children the possibilities of disease in the future is not a concern. There is a far more pressing cause of pain for these obese children on a daily basis. This pain is caused by discrimination by their classmates. Other children are much more direct and cruel then most adults when it comes to ridiculing the ?fat kids? they associate with. A first grader is likely to believe that thinner is better and young children will put these thoughts in to action by excluding their heaver classmate from their team or by calling them names.

This ridicule may cause a child to retreat from social interaction or to become a bully, using their weight to intimidate their tormentors. Either course of action only causes the child more grief and frustration in the long run.

Most children are not receptive to traditional restrictive diets. When they are told that they can no longer have the foods they crave, such as: white bread, candy, cookies, and ice cream they feel that they are being punished. It makes little difference how much time the parent spends trying to convince the child it is for their own good.

Children will often find access to the foods that are being denied to them at home with friends, relatives who don?t believe in diets or by spending their own money to purchase the sweets. If the child doesn?t have money they may begin to steal the foods they crave.

Many times when the child gains access to the forbidden items they will binge. 30% of girls and 16% of boys who were on a diet admitted to binging on foods that had been forbidden to them when they got the chance. Now the child not only feels punished by the parent, they have added guild to the emotional mix by defying their parents? orders or breaking the law. This guilt causes stress that may cause the child to act out in other more serious ways.

Insurance coverage is dependent on the therapist. Some therapists participate in network with medicare and other comercial plans while others are out of network. It is important to communicate with the therapist prior to the first visit to find out how your treatment sessions will be covered.

Tips to Help You Gain Weight

There are a couple of reasons why most people fail in their attempt to gain more muscle mass:

A. Improper diet.

Most people are not eating enough protein and eating too many simple carbs.

They are not stressing their muscles during each workout. You don't have to kill yourself, but you must subject your body to out of the ordinary stress each workout to grow muscle.

B. They lack consistency.

They do not stay focused throughout the entire 12-week period. If they don't see results immediately, they get discouraged and quit. You have got to stick with your plan. No program will work for you if you are not consistent.

To get results, you have to be willing to do whatever it takes, and work as hard as necessary and you have to be consistent. Your body responds to consistency. Sometimes it may get to the point of obsession, but it has to be that way for you to reach your goal.

Here's some basic information and things you should be doing to help you bulk up:

1. To gain weight you must eat more calories than your body burns off, so EAT MORE!!!!!! The most important thing that I cannot over stress is that you need to eat to gain weight. You need to eat like you've never eaten before. (but not junk food like donuts and chips or candy).

Start eating six meals per day (space them out to about once every 3 hours).

2. Increase your protein intake and reduce your simple carbohydrate intake. Without protein your body cannot build new muscle

3. Keep your workouts under one hour. Short and intense!

4. Concentrate on free weight exercises that work the large muscle groups. The best weight training exercises for building mass are the simple ones. For mass, stick with compound free weight exercises like squats, deadlifts, bench presses, barbell rows, pull ups and bar dips.

5. Use heavy weights and low reps, rest 3 minutes between each set.

6. Do only 2-3 exercises per body part.

7. Split your workout. Since you have a very high metabolism like me, you need to train with more intensity, but less frequently.

Day 1: Chest, shoulders and triceps

Day 2: Rest

Day 3: Back, and Bicep

Day 4: Rest

Day 5: Legs and abs

Day 6: Rest

Day 7: Rest

8. Increase you water intake. A good formula for this is to multiply your bodyweight by .66 to get the required number of ounces per day.

9. Use nutritional supplements. If you can't afford too many products, just stick with the basics; like whey protein. If you can't afford whey protein the next best thing is egg whites.

Then, as you grow up, and grow old, how you invested your 'trust' along the way, directly affects how well you're able to capitalize on the investment Life has made in you.

    Because our energy needs to last a lifetime, one of the first lessons we need to learn as children is how to replenish, and add to, our original supply of energy as we go about the business of living. Unfortunately, like the folk wisdom related to 'thrift,' the knowledge pool related to 'health' has more or less dried up too. Though, to a large extent it still operates in how we relate to some things in our lives. Take your car for example: it's generally understood that you need to keep your car fueled and maintained if it's going to carry you where you need and want to go. It's useful to apply this logic to your body too.

    So given the importance of our trust fund, it's essential to know how to invest and manage it in ways that deliver the return of a lifetime. And a key principle to remember in managing your 'trust fund is this: 'Avoid spending your principal!' or you'll be broke sooner rather than later.

    Oxygen Masks

    Oxygen masks are effective oxygen delivery device which is used mainly to supply oxygen from a storage tank to the lungs. Usually, this device is a mask or covering of the nose and mouth which is made of plastic silicon and rubber. Many people make use of this nowadays most especially medical care providers, aviators, medical researchers and hyperbaric chamber and other patients. Because of this, a wide variety of styles are now available for oxygen masks.

    There are four available basic styles of oxygen masks. These include the simple facemask, the venture mask, the partial rebreather mask and the non-rebreather mask.

    The simple facemask is the most commonly available oxygen mask to the public. This has a number of vents on both sides and can deliver 35-40 percent of oxygen. However if the oxygen flow increases to 10L/min, this can deliver up to 50 percent oxygen. The disadvantage with this type of oxygen mask is that this seals poorly and has large ventilation holes, thus oxygen flow is diluted with air.

    The venture mask, on the other hand, is an oxygen mask that uses a mechanical venturi effect in order to increase the flow rate of oxygen into the mask. Usually, this is used online in a clinical setting and is not advisable to be used in the field since it only delivers supplemental oxygen which is only about 24 to 28 percent.

    Another type of oxygen mask is the partial rebreather. This type of mask, which is often called as medium concentration oxygen delivery mask, almost looks like the non-rebreather mask but it does not have a one-way valve between the mask and the reservoir bag. This delivers almost 40 to 50 percent oxygen, and can increase up to 60 percent.

    The last type is the non-rebreather. From the four types, this is the one that can deliver up to 90 percent of oxygen. This makes use of valves on both sides in order to prevent air and patient exhalation from diluting the oxygen in the reservoir bag. The valves open when the patient inhales and breathes in oxygen. However, masks of this type with valves on both sides are prescription masks only.

    These are the different styles of oxygen masks. All are able to deliver the amount of oxygen needed. However, before choosing one, it is still best to seek medical advice especially if you are thinking about getting a non-rebreather oxygen mask.

    Exercises for Relief of Foot Pain

    Weakness in the foot can cause falling arches, bunions and hammer toes. These are potentially very painful. However, you can get a lot of relief by a few easy exercises.

    There are two arches in the foot. One runs across the foot on the bottom at the joints where the toes join the foot. The other is at right angles to the first and runs from the heel to the base of the first toe. These arches are supported by ligaments and muscles. There is not much you can do to strengthen ligaments, but exercising the muscles that support the arch are an important method to relieve your foot pain.

    When they fall, your arches stretch out and become irritated. You can get pain directly from that. But, bone spurs can also develop and they can be very painful as well.

    Bunions are formed when the sideways (transverse) arch falls. Then it pushes the sides of the joints outwards where they rub against the inside of the shoes. This builds up a callous.

    When the lengthwise (longitudinal) arch falls, the change in the foot pulls up the base of the toes. This puts more pressur on the bottom of the base of the toes and the tops of the middle of the toes rubs on the inside of the shoes. This will cause increased pain and callous formation.

    There are muscles that attach to different parts of the arch and tend to pull it up. When you strengthen those mucles you help keep the arch up. One easy exercise can be done while you are watching television. Simply drop a few quarters on the carpet in front of your chair and practice picking them up with your foot. This will help keep the foot muscles strong and limber.

    A second exercise is also easy. That is simply trying to 'make a fist' with your foot. Tighten up the muscles and hold it for the count of 5 and then release. As you get stronger, hold it for longer and longer. Do repetitions. As you get stronger, you should start getting some relief. The final exercise is a bit hard to explain, but here goes. You need to roll your foot around and then go back the other way. So, you start by pulling your foot up. Next rotate it outwards - then down. Finally rotate it inwards and go back to the starting position. The trick is to do it against resistance. That way you will be building up the muscles in your lower leg that pull up on your arches.

    Another type of pain may seem like it is causes by falling arches, but is actually from the tendons in the lower leg being tight.

    You can improve this type of pain by stretching out the back of the lower leg. Do this by putting your foot up on a low stool while you are standing. Hold the foot so the big toe is pointed straight up. When you are in this position, pull your foot up so your big toe is trying to point toward your head. You should feel stretching in the back of your leg. After doing this for a few days, you should start feeling some relief of your foot pain. Even if you are not having pain, these are great exercises to do to keep you from starting to get pain. So, you should be doing them even if you don't have foot pain right now.

    If you still have pain after doing these exercises, you may need some arch supports in your shoes. If your problem is mild, you can buy them at any drug store. If the arches are very bad, your doctor may need to prescribe special ones.

    Give the exercises a try. But if you still have no improvement in your pain after a week or so, you should see your doctor.

    How To Medically Treat Nail Fungus

    Treatment Of Toe Nail Fungus

    Medical Care:

    Several years ago the medical management of OM was limited to topical therapy and 2 unreliable systemic drugs: griseofulvin and ketoconazole. Topical therapy is beneficial only for mild cases involving the very distal nail plate. The use of griseofulvin and ketoconazole is plagued by high relapse rates (70-85%), prolonged treatment regimens (10-18 mo for toenails), constant laboratory monitoring, and numerous adverse effects. The introduction of newer oral agents has revolutionized the medical treatment of OM and reduced potential adverse effects and drug interactions. As the rate of recurrence remains high, even with newer agents, the decision to treat should be made with a clear understanding of the cost and risks involved as well as the risk of recurrence..

    Topical antifungals

    The use of topical agents should be limited to cases involving less than half of the distal nail plate or for patients unable to tolerate systemic treatment. Agents include amorolfine (approved in other countries), ciclopirox olamine 8% nail lacquer solution, sodium pyrithione, bifonazole/urea (available outside the United States), propylene glycol-urea-lactic acid, the imidazoles, and the allylamines.

    Topical treatments alone are generally unable to cure OM because of insufficient nail plate penetration. Ciclopirox solution has been reported to penetrate through all nail layers but has low efficacy when used as monotherapy. It may be useful as adjunctive therapy in combination with oral therapy or as prophylaxis to prevent recurrence in patients cured with systemic agents.

    Oral therapy

    The newer generation of oral antifungal agents (itraconazole and terbinafine) has replaced older therapies in the treatment of nail fungus. They offer shorter treatment regimens, higher cure rates, and fewer adverse effects. Fluconazole (not approved by the Food and Drug Administration [FDA for treatment of nail fungus) offers an alternative to itraconazole and terbinafine. Derivatives of fluconazole may also be available soon. The efficacy of the newer antifungal agents lies in their ability to penetrate the nail plate within days of starting therapy. Recent evidence shows better efficacy with terbinafine than other oral agents.

    To decrease the adverse effects and duration of oral therapy, topical and surgical treatments may be combined with oral antifungal management.

    Surgical Care:

    Surgical approaches to nail fungus treatment include surgical nail avulsion and matrixectomy by chemical or mechanical means. Chemical removal by using a 40-50% urea compound should be reserved for patients with very thick nails or for those who may not tolerate mechanical avulsion.

    Removal of the nail plate should be considered an adjunctive treatment in patients undergoing oral therapy.

    A combination of oral, topical, and surgical therapy can increase efficacy and reduce cost.

    Activity: Activity does not need to be limited during treatment, but patients should be educated about avoiding direct contact with high-risk areas in public places.

    Internet journals are increasing daily. People use them to communicate with the world at large, political candidates use them to spread their message, even companies have started using them for advertising.     Why should you keep a journal? Well it lets you get things out that you may have difficulty saying to other people. There are journals that are kept on your computer, if you prefer typing. You can type it out, save it to a hard drive. If you have a laptop, you even have the portability of a hand-written journal. Unfortunately, you still have issues regarding keeping the journal. A virus on your computer, or a power-failure and you can lose what you wrote. It can also send your journal onto the internet without your choosing. Finally, there are internet journals. They are already on the internet, but could be read by anybody. Even if you lock them, they can be read unintentionally.

    Obviously, no solution is perfect, but there are advantages to having a journal. By keeping one, you can make sure that you are tracking your road to health and keep yourself encouraged in many ways.

    Excessive high blood pressure can lead to a stroke. A stroke occurs when a blood vessel, which supplies blood to the brain, bursts or is blocked by a blood clot. Within minutes the nerve cells in that area of the brain are damaged, and they die within a few hours. As a result, the part of the body controlled by the damaged section of the brain cannot function properly. Before having a stroke you may have one or more transient ischemic attacks (TIAs), which are a warning signal that a stroke may soon occur.

    The symptoms of a stroke can be any or all of the following: Numbness, weakness, or paralysis or the face, arm, or leg, especially on one side of the body. Vision problems in one or both eyes, double vision, or loss of vision. Confusion, trouble speaking or understanding with trouble walking, dizziness, loss of balance or coordination, and a severe headache.

    Remember to monitor your blood pressure, and to speak to your doctor if you have any of the symptoms mentioned above. If you have a history of high blood pressure be pro-active and take your blood pressure often. Blood pressure monitors can be purchased at pharmacies. You can also have your blood pressure taken at a pharmacy at no charge.

    Just What Is A Carbohydrate Anyway?

    During a recent discussion with my father about low carb diets, he pointed to the butter on the table and said ?is that a carbohydrate?? when I started to explain, he then pointed to the sugar and said ?well, what about that??

    Unfortunately, you cannot just point to a food and label it a ?carbohydrate? as each food is made up of protein, carbs and fats in varying degrees. Some foods are almost all carb, and some, like meat, eggs and fish have none.

    To make matters even more complicated, there are simple carbohydrates and complex carbohydrates. For matters of nutrition, you should always pick the complex carbs which have a low glycemic index.

    The simple carbohydrates include foods like desserts, sugar, white bread, candy. Typically, these foods have a little nutritional value. Though they may taste good, they are basically empty calories and are digested quickly causing a high blood sugar spike and then a quick drop off. This is the reason these foods don?t really satisfy your hunger and you get cravings for more shortly after having eaten them.

    By contract, the complex carbohydrates are extremely nutritious, these are foods like legumes, whole grains, vegetables and most fruits. They are loaded with fiber, antioxidants, vitamins and minerals. Calorie for calorie, complex carbohydrates will fill you up more and leave you full longer then the simple carbohydrates.

    When choosing which carbohydrates to put on your plate, you should try to load up on whole grains, fruits and vegetables but if they are not available in plentiful enough amounts, you should try to balance your simple carbs with your complex carbs and proteins. Use the simple carbs as more of a side dish - maybe filling ? of your plate with them and fill up on the lean protein and complex carbs. This will have a net effect of a more nutritious and filling meal as well as leave you satisfied longer.

    Influenza Epidemic and Methicillin Resistant Staphylococcus Aureus (MRSA)

    Specialization among infectious disease experts has worked against an in depth understanding of the complexities of different pathogens working cooperatively to cause serious and even fatal human infections. The anticipated human epidemic of an avian derived strain of H5N1 influenza should be a wake up call to address this important question. Influenza can render the body particularly susceptible to certain types of bacteria that can thereby flourish; and could potentially become the driving force of a continuing worldwide pandemic. Prominent among these bacteria, are toxin producing Staphylococcus aureus, some of which can be resistant to various antibiotics, including methicillin.

    Pioneering research during the 1918 influenza epidemic clearly identified a virus component as the initiating cause of illness. Yet there are ample indications that bacteria were responsible for the gravity of the secondary pulmonary complications, and the common causes of death. The idea of a mixed infection is contained in the oft quoted letter written by a military physician in 1919.

    Camp Devens is near Boston, and has about 50,000 men, or did have before this epidemic broke loose?. This epidemic started about four weeks ago, and has developed so rapidly that the camp is demoralized and all ordinary work is held up till it has passed?.. These men start with what appears to be an ordinary attack of LaGrippe or Influenza, and when brought to the Hosp. they very rapidly develop the most viscous type of Pneumonia that has ever been seen. Two hours after admission they have the Mahogany spots over the cheek bones, and a few hours later you can begin to see the Cyanosis extending from their ears and spreading all over the face, until it is hard to distinguish the coloured men from the white. It is only a matter of a few hours then until death comes, and it is simply a struggle for air until they suffocate. It is horrible. One can stand it to see one, two or twenty men die, but to see these poor devils dropping like flies sort of gets on your nerves. We have been averaging about 100 deaths per day, and still keeping it up. There is no doubt in my mind that there is a new mixed infection here, but what I don't know.

    Among the bacteria commonly cultured were Pneumococcus, Streptococcus and Staphylococcus. While H1N1 influenza virus has been retrieved from victims of the 1918 epidemic, no formal study has been reported of possible toxin producing bacteria from this period.

    The vast majority of bacteria are essentially harmless to mankind. Bacteria can, however, become infected with their own sets of viruses, some of which can transfer toxin producing capacities to otherwise relatively harmless bacteria. Bacteria viruses can also transfer the capacity of bacteria to resist certain types of antibiotics. The combination of toxin producing capacity with antibiotic resistance is now occurring, especially among Staphylococcus aureus. Of great concern is a toxin complex known as Panton-Valintine-Leucocidin or PVL. This toxin can easily incapacitate the host inflammatory response by directly killing white blood cells (leucocytes). The toxin can also destroy otherwise healthy tissues if the bacteria producing the toxin can gain entry into the tissues. The PVL toxin was originally detected in antibiotic susceptible bacteria.

    Antibiotic resistance among bacteria is a consequence of genetic selection of the surviving bacteria in patients treated with various antibiotics. These resistance genes become commonplace especially if carried by bacteria infecting viruses. The emergence of these bacteria is mainly seen in hospitals and other healthcare facilities. Indeed, a major risk factor from hospital admission is acquiring a multiple antibiotic resistant bacterial infection. The phenomenon is well documented among Staphylococcus aureus. Originally highly susceptible to penicillin type antibiotics (known as beta-lactams and commonly represented by the antibiotic methicillin), many hospital acquired Staphylococcus aureus are now methicillin resistant. In addition, they are resistant to many other types of antibiotics commonly used in the hospital setting. Examples of resistance to the toxic antibiotic of last resort (vancomycin) are now showing up in Staphylococcus aureus and other bacteria in certain hospitals.

    The PVL toxin producing Staphylococcus aureus has started on the path of becoming antibiotic resistant. At present most community associated isolates are resistant to methicillin (CA-MRSA). In time, they will surely become resistant to a wider range of antibiotics by simply exchanging genetic information with hospital associated bacteria (HA-MRSA). The only barrier left to widespread severe infection, is the normally non-tissue invasive quality of Staphylococcus aureus. Influenza infection can provide such an opportunity by destroying the cells lining the air passages. Examples of fatal illness from a combination of regular influenza with CA-MRSA have been reported with little emphasis of a portend of what could occur in the face of an influenza epidemic and multiple antibiotic resistant, PVL toxin producing bacteria. Worse still, this is but one example of the enormous risks posed by pathogens teaming up in a biological warfare against mankind and his animals.

    What should be done? Foremost is an all out attack on the emergence of toxin producing and/or multiple antibiotic resistant bacteria. Financial incentives exist for developing additional antibiotics to replace those for which resistance has developed. This approach should give way to a more common sense approach of preventing infection through decontaminating areas in which harmful bacteria reside.

    A lack of awareness of decontamination strategies among Government and public health officials is apparent in their recommendations of simply using alcohol hand washing and short acting oxidizing agents such as bleach. Far more preferable is to use agents such as phenols and their derivatives that can retain antibacterial activity over many months. Surveillance for toxin producing and antibiotic resistant bacteria need to be in place in hospitals and settings where large numbers of individuals assemble. Examples include jails, schools, churches, sporting amenities, and workplaces where skin trauma is likely to be encountered. A comprehensive hygiene program, such as the one offered by Preventec inc., in Atlanta GA, should be instituted at such facilities and its effects monitored.

    The benefits of this type of program may well extend to other types of infectious agents, including viruses and fungi. An additional complication of the 1918 influenza epidemic was the subsequent occurrence of neurological diseases, including a Parkinson-like syndrome known as encephalitis lethargica. Underappreciated research implicated a herpes-like virus in this illness. Swine flu vaccination triggered another set of neurological diseases, most prominently a Guillain Barre syndrome that are also consistent with a virus activation process. Viruses that are not effectively recognized by the immune system are prevalent within the community.

    Termed stealth adapted viruses, they undoubtedly contribute to outbreaks of community acquired infectious illnesses with prominent neurological and/or psychiatric manifestations. Bacterial genes have been identified in cultures of stealth adapted viruses and atypical bacteria have been isolated from stealth adapted virus infected patients.
